Math Department Policies for Online Exams
These are provided here for your convenience, but these should be also contained in your course syllabus.
Code of Academic Integrity
The Code of Academic Integrity will be enforced in all areas of the course, including, but not limited to, assessments and online assignments. For more information about the Code of Academic Integrity policies and procedures, including information about your rights and responsibilities as a student, see Academic Integrity.
Students are encouraged to share intellectual views and discuss freely the principles and applications of course materials. However, graded work/exercises must be the product of independent effort unless otherwise instructed. Students are expected to adhere to the U of A Code of Academic Integrity as described in the U of A General Catalog. See: https://deanofstudents.arizona.edu/policies/code-academic-integrity
Students found to be in violation of the Code are subject to penalties ranging from a loss of credit for work involved to a failing grade in the course, and the risk of possible suspension or probation. Failing to observe the following rules of academic integrity established by the instructor for this course is considered a potential violation of the Code of Academic Integrity and will be handled accordingly.
Rules:
- Students are not permitted to use outside resources such as:
- Artificial Intelligence (in apps like ChatGPT, Photo Math, Math Way...) on assessments.
- A second monitor during proctored assessments
- Use of a cellphone or unauthorized devices such as smart watches or handheld calculators during proctored assessments
- Other unauthorized resources/materials during proctored assessments
- Students may not submit other people’s work as their own for assignments or assessments.
Exam Attendance
Students who complete an exam AND follow all exam procedures will be marked “Present” for the exam and will be graded as indicated by the syllabus. If any exam procedure is not followed, the student will also be marked as “NOT Present” and will receive a score of zero for that exam. Students must follow ALL exam procedures in order to be marked “Present” for the exam. See the list below for all exam procedures.
Proctored Exam Replacement/Missed Exam Policy
The exam replacement policy states:
Your lowest midterm exam percentage score (including the possibility of a score of zero) will be replaced by the final exam percentage score, if your final exam percentage score is higher.
Note:
- If two or more of your lowest exam scores are identical, only one may be replaced.
- Any exam score that has been modified due to a violation of the Code of Academic Integrity where a report has been filed and upheld is not eligible for replacement.
Because this course includes a final exam replacement policy, any exam for which a student is marked not present will receive a grade of zero, and there will generally be no make-up exams. In the case of extreme and unusual circumstances surrounding a second exam for which a student is marked “NOT Present”, a make-up exam may be granted at the instructor’s discretion. Note: An unsatisfactory score on a previous exam does not constitute extreme or unusual circumstances.
If a make-up exam is granted, it must be completed within the timeframe specified by the instructor, generally within one week of the testing window.
Religious holidays listed on the following website and activity excuses will be honored according to the U of A Official Activity Excused Absence policy. Students are responsible for arranging in advance to make up missed work or material in a timely manner. For more information, see the Religious Accommodation Policy website.
Testing Procedures Set-Up
The procedures below must be followed, or an Honorlock proctor may interrupt you during your exam and ask you to correct it. Your exam timer is not paused during these interruptions, so it is to your advantage to have a proper set up.
Designated Testing Space:
- Must have a good internet connection and be at a desk or table
- Should have anything that you don’t want recorded removed or covered
- Must remove any visible writing on the desk or walls
- Must NOT have any loud music, television, or other distractions playing in the background
- Must NOT have other people or parties near the testing environment
To prepare testing equipment students must:
- Download the Google Chrome Browser on their computer
- Check for updates before each assessment
- Check that the External Webcam is working and positioned to capture a side-view of your space, which shows your face and the entire desk area within the video frame
External Side-view Webcam Video Feed captures the following:
- Head and hands
- Side view of work area that includes the space to the left and right of the computer, mouse, and keyboard.
- Work area where writing is occurring

Note: You may not use the camera in the computer for this purpose.

Before the Honorlock session starts, students must:
- Have a physical CatCard or Government-issued Photo ID out and ready.
- Note: Honorlock is not compatible with digital CatCards.
- Remove hats and hoodies
- Remove all wearable technologies. This includes, but is not restricted to smart watches, earbuds, headphones, and the like.
- Have loose-leaf paper or provided written work template (No notebooks are allowed)
- Have writing utensils like a pen or pencil/eraser
- Have cell phone
During the session students must:
- Maintain proper school-environment attire
- Stay in the testing window the entire time; head, hands and papers should be visible throughout the exam
- Cell phone should remain face down in view of the camera until needed at end to submit written work.
Exam Procedures
(all 10 procedures must be followed in order to be marked "Present” for the exam)
At the start of the assessment, after you have authenticated through Honorlock and you see Question #1 in the exam:
- Ensure that external side-view webcam video feed is selected by opening the webcam preview window.
- Keep the webcam preview window open throughout the exam session.
- Make sure the angle of the camera shows the entire testing area within the preview window. This includes the desk area on both sides of your computer, mouse, and keyboard, with head and hands in view of the camera. All pages must remain in view of the camera throughout the exam.
- Make sure you are seated at a desk or a table that is cleared of everything except for allowed exam materials throughout the exam session.
- Ensure the lighting is bright enough so that your face and items on the desk are clearly visible (Check the preview window and adjust as needed; the camera view may show the room’s brightness differently than it appears to you.)
- Show cell phone and wearable technologies to the camera; then place them face down on the desk, in view of the camera throughout the session.
- Show each blank loose-leaf page or written work booklet, front and back, to the sideview camera.

At the end of the assessment, before you exit the Honorlock session:
- Show your used pages of written work (each page individually, front and back) to the camera.
- Scan/upload your work to Gradescope/D2L Assignments (as specified in syllabus).
If you become disconnected from Honorlock or have a technical issue:
The possibility of a make-up attempt for extreme and unusual circumstances will ONLY be considered for the second exam that is marked as “NOT Present”.
Situations like the following will be handled individually by the instructor:
- Power outage
- Loss of internet connectivity
- Sudden severe illness for you or your family that interrupts your exam time or keeps you from starting
In order to be considered for a make-up exam the following steps must be followed:
STEP 1: Use the Live Chat tool on the Honorlock website to reconnect to the exam, as you will be locked out of it. Using the Live Chat also provides a way to document your attempt to reconnect, since your chat history is downloadable.
If you cannot reconnect:
STEP 2: Fill out the form from the Exam Procedures module in D2L indicating the date, time and description of the incident.
- You will be asked to provide documentation that supports your description, for example an outage map from the power company.
- Your instructor will be notified of the incident report and reach out to you to set up a meeting to discuss the situation.
A make-up opportunity may be granted at the instructor’s discretion. If a make-up opportunity is offered, it must be completed within the time span designated by the instructor (generally within 48 hours of the original testing window). Failure to do so may result in a grade of zero for the assessment.
